*For quantity please make sure it matches the number of dancers you have being evaluated. For example: If you have 2 dancers who are interested in being evaluated your quantity would be 2 and so on.*

If for any reason your dancer is unable to attend evaluations you will still need to sign up above. In person evaluation is highly recommended **
